The Incident Board.

Documented AI security incidents.

Methodology

The Incident Board follows AI security incidents: what happened, the boundaries that failed, the consequences and the response. Each record brings the explanation together with its chronology and supporting sources.

We distinguish public evidence, accounts from the organizations involved, and questions that remain unresolved. Official oversight separates incident-specific actions from wider policy context. Its review date is shown in each box; an absent response in the cited record does not establish inaction.

These are continuing editorial records based on the linked evidence. Coverage is selective, and unresolved questions remain visible. Record updates describe significant reporting changes; each brief states when its sources were checked.
