About
The Incident Board is an editorial project by Schuler Media LLC in San Francisco.
We document AI security incidents: what happened, which boundaries failed, who was affected, and how companies and public institutions responded.
Evidence first
Each incident brief brings together an overview, reported impact, an account of events, a timeline, the failure mechanism, sources, consequences, vendor response, official oversight, open questions and a record of significant updates.
We distinguish documented facts from claims made by the organizations involved. Unresolved questions stay visible. A company’s statement establishes its position; it does not automatically confirm every claim about an incident.
Living records
An incident’s timeline describes events. Its record updates describe material changes to our reporting. Dates identify when information was reviewed, and an update does not imply that every source has been checked again.
Our four-level impact rating is an editorial assessment based on the available evidence. It is not a technical vulnerability score or a prediction. Read the assessment methodology.
Scope
Coverage begins with two incidents and will grow as we research further cases. This is a selective editorial record, not a complete catalogue of AI failures.
